This application is the first step in the adoption process for one a UYR foster dog. All of this information is important so please be as accurate as possible. If a question does not apply to you, you may enter .N/A. It is only necessary to fill out one adoption application, regardless of how many dogs you are interested in. Press the .Tab. button to go to the next question. Do not press .Enter. unless it is within a multi line text box.

Our webmaster set up the form so that ALL FIELDS must be answered, even if the answer is N/A. If you submit it, and then you see the form again on your screen, scroll down and you will see IN RED. "This is a required field!". The missing field is the field/question ABOVE the aforementioned message. Check that and make SURE you have responded to all questions/fields. Also remember to type your name at the bottom where the signature normally is.

NOTES: UYR does not adopt dogs to individuals under the age of 21.

UYR does NOT ship our dogs to adopters. If you are selected to adopt one of our fosters that isn't in your area, you will be required to travel to the foster dog’s home, either by car or plane, to pick up your dog. Some airlines permit dogs to travel with an individual in the cabin as carry-on baggage. You will need to purchase a pet ticket.

I understand that proper food and veterinarian care may be costly, and I am able and willing to meet the needs of my adoptive dog. I understand the requirements and grant permission for Home Checks at random, both prior to and following adoption. I submit that if a Home Check reveals falsification or misrepresentation of any facts on my part, United Yorkie Rescue reserves the right to either refuse adoption or to nullify the adoption and reclaim said dog from my premises without reimbursement of the adoption donation. I release United Yorkie Rescue from any liabilities I may incur from the adoption process.
